Individuals selected for this program are based on nurse specialty. You must have verifiable work experience in the last two years in one of the following areas to be eligible for reimbursement.

- Case Management
- Cath Lab
- Chemo Certified
- Interventional Radiology
- Endoscopy
- CVICU- Must be able to recover fresh open hearts
- PICC Lines- Must be able to insert PICC lines
- CVOR


This program is limited to a select nursing specialty group.  

If you apply for a new nursing state license or renewal in Oregon between May 15 and June 3, 2024, license application fees __up to $350__ will be reimbursed.  

Reimbursement and expenses paid includes the application fee, license verification and background check fees. 

__USN will not reimburse for postage, transcripts or coursework fees.__ 

Qualified applicants will be reimbursed within 4-6 weeks __upon proof of an ACTIVE Permanent verifiable online Oregon license.__  If you have never completed an assignment with Fastaff or USN, you MUST provide a W-9 IRS form with your receipt to be reimbursed.

*__You must obtain an active Permanent Oregon license AND be submitted to one of the Oregon specialty positions listed above in order to receive reimbursement.__ * 

All Oregon license receipts must be emailed to __USNLicensing@usnursing.com__ by __Monday, June 3, 2024__ to qualify for reimbursement.

### Verification of licensure

- Submit verification of original state licensure.
- Submit verification of most current or recently practiced state license.
- If you are licensed in a state that does not participate in Nursys, contact the board you are licensed by for proof of verification instructions.

### Background check

- Background Check Fee:  $70.50
- The OSBN contracts with Fieldprint Inc. to collect digital fingerprint images from applicants for the background check. __AFTER submitting an application for licensure,__ visit Fieldprint Inc's website to schedule an appointment to have your fingerprints taken and transmitted electronically. Fieldprint Inc. offers services nationwide.

- All fingerprints are processed through the Oregon State Police (OSP) and the FBI. A history of criminal activity will be reviewed and may result in the denial of your application.

### Official Transcripts

- Request nurse transcripts to be sent directly to the OSBN.  Documents can be mailed or emailed to osbn.transcripts@state.or.us
