Standard File Requirements

Below are U.S. Nursing's standard file requirements. Please note - these may be adjusted based on a facility’s specific requirements. Nurses who arrive on site with all documents completed will finish the induction process quicker than those who do not complete their paperwork ahead of time. 

These documents must also be hand-carried to the job action. 

Skills Checklists

Skills checklists are completed via our nurse portal or via a Prophecy link that will be sent to you by one of our credentialing specialists. To access the nurse portal click here. If you do not have a nurse portal user ID and password please contact a strike team member at 800-726-8773.

Certifications

BLS, ACLS, PALS, NRP, FHM, ENPC, TNCC, ONC, MAB, NIH (as required by specialty).

State Nursing License

Copy of current nursing license in the state where the job action is occurring.

If the striking state is compact, then you must provide a copy of your compact nursing license and proof of residency in that compact state.

Physical

Physical within one year. Download Physical form.

Tuberculosis

One tuberculosis skin test (TB) within one year. Quantiferon within one year is also accepted. If TB is positive or if you have a history of a positive TB, you must provide a Chest X-Ray within 5 years and TB Questionnaire within one year.  Download TB Questionnaire form.

Rubella

Proof of positive titer or one vaccine.

Rubeola

Proof of positive titer or one vaccine.

Mumps

Proof of positive titer or one vaccine.

Varicella

Proof of positive titer, documented history of disease, or one vaccine.

Hepatitis B

Proof of positive titer, completed vaccination series, or declination.  Download Hepatitis B Declination form.

TDaP Vaccination

Vaccination within 10 years, or declination.  Download TDaP Declination form.

Flu Vaccination

Vaccination for current season, or declination.  Download Declination form.

Evaluation

One evaluation within specialty from a RN or higher within two years.  Download Reference Evaluation form.

These forms are required ONLY after you receive an OFFER for the job action

I-9 Form

Fill out Section 1, but leave Section 2 blank. A U.S. Nursing representative will fill out Section 2 during Induction. A Credentialing Specialist will instruct you further on how to complete this requirement, closer to the time of the strike.

Background Check Form

This will be sent out to anyone who has not previously signed a form. This form gives U.S. Nursing permission to run a background check as required for employment.

The following background checks are conducted: SSN Verification, Enhanced Nationwide Criminal Search (7 years), FACIS Level 3 Search (includes OIG and SAM), National Sex Offender Database Search.

Drug Screen

Take a 10-panel drug screen through US Nursing, unless you have a current drug screen on file within the last 1 year with Fastaff/US Nursing. A Credentialing Specialist will instruct you further on how to complete this requirement, closer to the time of the strike.
