After two years of expiration, will be required to apply as new applicants for licensure. To restore or reinstate a license, email or call the board to request a restoration or reinstatement application Inactive status 2 yrs or more – Provide proof of current licensure in another state with employment verification of at least 520 work hours within 2 years.
Evidence of status form required if license lapsed on or before November 1, 2007. All licenses expire on June 30 of each odd-numbered year regardless of issuance date
