*Temporary permits are issued while other required docs are in process of receiving a permanent license. Additional necessary steps to receive a permanent license MUST also be initiated.

Reinstatements:

  • Application must be notarized
  • Lapsed within 5 years – Proof of 30 hours of approved continuing nurse education within 2 years
  • Lapsed 5 years or more – Verification of current active license and proof of 30 hours of approved continuing nurse education within 2 years

May apply for a 120 day temp permit to complete the 30 hours of continuing education.
